Royal Cornwall Hospitals NHS Trust staff operate in fast-paced, high-demand hospital environments with limited opportunity for extended recovery.
During the programme period, many teams were also navigating organisational change, role uncertainty, and restructuring. Stressors were predominantly systemic rather than individual.
The aim of the programme was not to remove pressure, but to strengthen staff capacity to regulate and recover within real working conditions.
The Approach:Â
We delivered a six-week online, physiology-based breath and movement programme designed to integrate into acute hospital settings.
The programme included:
- Weekly structured modules
- Education on breathing mechanics and autonomic regulation
- Practical focus on sleep physiology, attention, and movement
- Short, flexible practices (5–15 minutes) designed for use during shifts
The emphasis was on usable physiology, techniques that could be applied in real time, not added to already full schedules.
Evaluation Measures:
Baseline and post-programme data were collected as part of a Quality Improvement (QI) evaluation.
Measures included:
- Mood and focus at work
- Confidence in using breath and movement techniques
- Feasibility under hospital workload
- Motivation to continue practice
- Overall acceptability and recommendation
As a QI evaluation, findings focused on feasibility, acceptability, and perceived impact rather than formal statistical testing.
Results
Confidence Increased
All post-programme respondents (100%) reported moderate to high confidence in using breath and movement techniques independently, a clear upward shift compared with baseline confidence levels.
All participants also reported motivation to continue using the practices beyond the programme.
Improved Emotional Regulation
Participants described greater awareness of stress responses and increased ability to regulate in the moment:
“I take more time to stop and breathe before replying.”
“More awareness of breathing when stressed.”
“Remembering to breathe gives a moment of calm.”
“When I consciously use breathwork, it calms other symptoms.”
Better Equipped Under Ongoing Stress
Importantly, external pressures remained. However, participants described feeling more capable of navigating them:
“Despite being under more stress currently (work related), I feel more equipped to manage it.”
“I feel I have a resource at hand that I can use.”
Focus & Cognitive Support
Functional improvements were reported even when mood changes were modest:
“Extended focus in the afternoons when I usually dip.”
When mood and focus were considered together, the majority of respondents reported slight improvement.
Honest Reflection
Barriers remained for some participants, including workload pressures and life events. Challenges were largely contextual rather than related to the intervention itself.
Feedback highlighted the value of structured sessions, instructor guidance, and clear physiological explanations in supporting engagement and confidence.
Summary:
The programme demonstrated:
- Â Feasibility within acute hospital settings
-  A clear upward shift in confidence (100% moderate–high post-programme)
- Â Increased intentional use of breath and movement during real-time stress
- Â Strong acceptability despite ongoing organisational uncertainty
The shift did not remove pressure, it strengthened individuals capacity to regulate within it.
