Referral Pathway

A simple, supportive journey from referral to confident self-management.

Referrals are suitable for people experiencing stress, disrupted sleep, MSK discomfort, overwhelm, fatigue, low confidence or difficulties coping with daily demands.
We work with people whose challenges affect daily functioning but do not require medical or psychological treatment.

Until we are added to local directories, referrals can be made directly by contacting us via email or using the referral form below.

We accept referrals from:

  • GPs and primary care teams

  • Social Prescribing Link Workers

  • Health & Wellbeing Coaches

  • Care Coordinators

  • Mental health practitioners

  • Community & VCSE partners

  • WorkWell coaches

  • Self-referral (where PCNs allow it)

All staff are DBS-checked, safeguarding-trained and trauma-informed.

First Contact

Within a week, we get in touch with the person to:

  • introduce the programme

  • understand what matters to them

  • explore pressures, barriers and daily functioning

  • check for safety, suitability and risk

  • identify whether group or 1:1 support is the best fit

Referrers receive a brief update once contact has been made.

First Session

In the first session, participants are introduced to simple breath and micro-movement practices that ease tension and reduce overwhelm. They experience a small shift straight away and learn one or two strategies they can begin using the same day. This early improvement helps build confidence and engagement.

 

Tailored Support Pathway

People are matched to the support that suits them best:

Group Pathway (4–8 weeks)
For people managing stress, poor sleep, MSK tension, fatigue, loneliness or low daily functioning.

1:1 Physiological Support
For individuals with very low capacity, complex stress, chronic pain or difficulty engaging in groups.

Combined Pathway
Beginning with 1:1 support and moving into a group as stability improves.

Supported Self-Management Toolkit
Simple practices to use between sessions that build confidence and activation.

Review & Personalised Care Alignment

At the end of the pathway, we explore:

  • what has shifted

  • current pressures and priorities

  • barriers to daily functioning

  • readiness for community activities or personalised care planning

  • next steps in their support journey

A short update is shared with referrers to support ongoing personalised care conversations.

Ongoing Support & Integration

People can stay connected through:

  • maintenance groups
  • online micro-practices
  • follow-up check-ins
  • the Small Shifts community
  • re-referral during flare-ups if needed

We keep link workers updated at key points to ensure smooth, joined-up support.
